I will investigate if the codes you mentioned affected the transformed B button at all (which really shouldn't happen at all, which makes the whole issue strange to me).
In the worst case scenario it's unsolvable I can apply a set force value for the proper B button when transformed in some of the codes. The value should always be 4D (which is the item ID for the Kokiri Sword), regardless of your obtained items / masks / equipment / quest status and transformed state. The regular B button for Hylian Link ranges from 4D (Kokiri Sword), 4E (Razor Sword), 4F (Gilded Sword) or FF (no item). The Set Item On B Button code only affects the regular B button for Hylian Link.
Interestingly enough, these are the B button RAM addresses for NTSC-U Collector's Edition Majora's Mask (PZLE01):

Transformed states usually can't use items, so if their B button is set to a non-usable item it reverts to the unusable bottle you were mentioning. Value 0x00 always uses the Ocarina of Time (which is usually by all transformed states, except Fierce Deity). Value 0x09 for Deku Link is the Deku Nut item, which makes sense since Deku Link uses them when flying. I suspect the game is hardcoded to let the Deku Nut item on the B button to cause Deku Link to shoot bubbles when on the ground.
0x50 is the value for the Fierce Deity Sword when in Fierce Deity form. It shares the same B button as Hylian Link. When transforming back into Hylian Link, it checks your sword in the Quest Status in order to revert the B button.
My brother theorized that I may have, by complete accident, done the swordless glitch as a Zora. That seems highly improbable, but is the only solution I can come up with, since I had Gold Dust on one of my C-Buttons.
