I've attached a screenshot of error. It consistently occurs around 30 min after starting up and playing The Last Story (haven't tested with any other game) and only occurs when running on D3D11. The game runs perfectly at 720p @ 60fps until the error, after which Dolphin crashes. Anyone experienced anything similar or have any solutions?

I could run it on OpenGL or Vulkan, but it just doesn't run as well.

Forgot to mention I was using a week old build (forget the exact one) at first, and the error still persists when using the latest build (5.0-8490).

Specs:

Windows 10 x64
16 GB RAM
Intel i7-8550U @ 1.99 GHz (Turbo @ 4GHz)
Nvidia MX150 2GB

Probably best to memorialize this with a bug report.

May also help if you could get a savestate right before it happens as well, but if it's random, fair enough. If you could get me a save file leading to an area where I could just idle for half hour that'd be fine too.

I currently don't have the time to create a bug report but I can confirm the bug is indeed random, apart from it consistently occurring ~30 minutes after playing, regardless of the area you are in. It occurs consistently in the first 30 minutes of the game as well so I don't think a savestate or save file will make a difference. If someone else could try it out with my settings on the emulator and see if they can replicate it that'd be great. It might indicate if the issue lies with the emulator or if it's just a problem on my end (or both).

I've attached the settings I'm using.

Does the crash still occurs if you disable Ubershaders?

The Last Story Dolphin wiki states ubershaders should be set to "hybrid" (I assume this is Asynchronous Ubershaders setting) to fix stutter but I'll try setting it to synchronous and report back.

Yup, still occurs. Although it lasted 40 minutes before crashing this time around.
