I just upgraded to Dolphin 4 and for some strange reason my games never start in fullscreen, I always have to switch to fullscreen myself.
The problem with this is that I sit back on my couch with my 360 controller and I don't have a mouse near me to click on fullscreen. This is normally not a problem because I have added dolphin to Steam and when I run steam in Big Picture mode I don't need a mouse and keyboard.

Do you have the fullscreen option checked in the graphics settings?

lol sorry, fullscreen was not checked. The problem this time was that I had installed Dolphin in German and I didn't find the fullscreen option in the german version it just says something like use the whole screen. I thought that option had been removed.
