Yeah, Auto + Limit by FPS does the trick for me, but I'm able to reproduce a lot of the other combinations you posted, Aelirie. I have to agree with paul here, give HLE audio the boot and use LLE audio instead. Your PC might not be fast enough for LLE audio however. You can try overclocking (do your research first!) or try using the VBeam Trick (google it) if you get stuttering audio under LLE.

120 vps at 30 fps has been confirmed since before I created this post.

My guess is that Virtual/Real XFB (which few games actually need) or VBeam Speed Hack are breaking it. Turn those off, if you have them on.
