You need to go to bluetooth settings (view bluetooth devices), and remove the NINTENDO_RVL_CNT device. Then add it again by 'add new bluetooth device' and hold down 1&2 on Wiimote. It will then install to windows.
Next, open dolphin and select wiimotes. Click on pair up wiimotes, and press down 1 & 2 on Wiimote. After about 3 to 5 seconds, it will say on the bottom right corner of screen near task bar "installing bluetooth HID device", and soon, "installation of bluetooth HID device was succesful".
Then click on 'refresh wiimotes' and it will shake. Then select 'real Wiimote' from the drop down list, and press OK. Then you will have assigned it as controller 1, and 1 light will be lit.
Make sure you have proper Bluetooth drivers installed. If the BT icon is not in your taskbar during startup, go to Start -> write services.msc -> Enable bluetooth support service to automatic mode. If you don't have this option, you haven't installed BT drivers correctly
ocean, u dont need to remove NINTENDO_RVL_CNT manually, if u click on the pair up button, it will remove the device automatically if it was already existent but unpaired.

But like i already said, u cant pair it up again, when it has lost its pairing during playing (like for example on empty batteries etc.).
In this case u have to close dolphin and either remove NINTENDO_RVL_CNT manually or just start up dolphin again and click the pairup button (press 1+2 button on ur wimote before u click the pair up button).
The pair up button will always remove the old unpaired deivce if one was already existent and then it will readd the device.
After u pressed the pair up button u just have to press the big button below the pair up button, Refresh Real Wiimotes. Et Voilá.

my wiimote is showing up in windows 7s bluetooth devices, it connects fine in dolphin and rumbles, but for the life of me i cant figure out how to use it in any game. I'm sitting at the "Push A!" start screen on mario kart wii and the first player light on the wiimote is light, i have it set to use real wiimote....
anyone?

Did you make sure you switched option from 'Emulated Wiimote' to 'Real Wiimote' after your controller rumbled and pressed OK to get the 1 light
Dont add your wiimote using ms own stack, i never did i just went to pair up wiimote directly through dolphin, windows popped up a message saying installing driver after which i clicked pair up again and its been working flawlessly ever since, also if it does disconnect while playing just click reconnect you dont need to pause the emulator or even exit and restart, its always reconnected fine for me.
Syill nothing for me. I think its my cheap off brand (NYKO) Wiimote or my VERY CHEAP Bluetooth adapter (thou Windows can see wiimaote and evrything else BT I've tried works). Gona try and buy a real Nintendo Wiimote.

I've got 2 real wiimotes and also experiencing the problems on the thread. More frustratingly, every time I restart my computer the pair up button doesn't work. The MS bluetooth stack I'm using (whatever comes with Win7 x64 by default) can still see both my remotes which I've renamed 1 and 2. However dolphin tells me I've no remotes connected in the wiimote plugin page. Pressing pair up does nothing.
The only way to recover from this is to go to the MS bluetooth devices page, remove both of the remotes. Then run dolphin again and do a pair up on each of the remotes in turn.
Having done all this (!) I have 2 working wiimotes with 1 led lit up on each for player 1 and 2 as you would expect. Then it's touch and go whether they will work when I run any game or whether it will just stick on saying "connecting" like others have mentioned.
Have tried various versions of dolphin but always the same experience. Currently using version 4992.

Far as I know this happens on every revision, and is a bug/feature yet to be fixed.
I've tweaked with all possible bluetooth and wiimote configs, after you restart PC, Dolphin
never recognises wiimotes automatically again.
So the only solution remains to, indeed, remove BT devices, and pair them up again.
nyko wand doesn't work? I was just about to buy it.
