The first square is Home Adress. The second square is Other Adresses. The rectangles are Erase All Information, PIN, and Notes from left to right.

The attached photo in the initial post is from a hidden section (Set Personal Data) in a Wii channel named in Japan: 「配送チャネル」.
That channel was very strange for anyone living in Japan at that time.
