Pretty sure this shouldn't happen... Any ideas on how to fix it?

Does this happen on the latest dev build of Dolphin? Have you tried reinstalling your graphics driver from Nvidia's website? (sometimes windows updates will break the driver)

Are you running a Gamecube bios/IPL file? if so... you are using a broken one... I've seen this happen in my game when I was using a broken IPL file.
