Dolphin: 5.0-6474 but I was on 5.0-6465 when it first started

I have also completely deleted that pack off of my computer (It was the first thing I did), if that helps

The MD5 hash is a valid one for the NTSC-U version according to
https://www.gametdb.com/Wii/GM8E01 so it's not a bad dump. I'll try mine.
BTW... It is cute because the Subtitle of Metroid Prime 3 is Corruption
Edit: Using Dolphin 5.0-6485 and drivers 389.77 I cannot reproduce this issue with these settings. Updating drivers.

Yes, it does still happen, in fact, it was even more of a mess because it kept spamming me with "invalid SYSCONFIG"
