I did that on Saturday.  I started with default settings except for 3xIR, 4xMSAA and 16xAF and everything was running well enough without the Skip EFB access hack.  I kept adding the rest of the settings I normally use one by one and testing and nothing seemed to slow the game down dramatically so I decided to revert to my regular user settings while keeping the hack disabled.  To my surprise, I was no longer getting the massive slowdowns but 

I'm still keeping the hack on for now because this is clearly not consistent behavior.  While I was doing my previous tests, the slowdowns were there with days and restarts between attempts.  I don't know how much of this is system issues but Skip EFB Access From CPU seems to be the only surefire solution in my case.
Skip EFB Access to CPU can get significantly slower at higher resolutions. I'm guessing the MSAA is breaking the effect anyway so having it on/off doesn't matter.

Just to make sure I'm following you:
Do you mean 4K type resolutions?  I play at 3x.
What effect is MSAA breaking?
