I fixed it. Just go into the graphics options, and under "general" there should be a "backend" option. Change that to "Direct3D" and everything should work fine.

I've done what you said should have worked and its still doing it.
swtiching to d3d won't fix the problem reliably. like i said, it's a known issue and a minor inconvinience, there is no reliable fix known
I'm having this issue, every time I load dolphin, not the first. It just refuses to play any of my games from my Ext HDD. If I move them to my Local Disk C it will play them. Then if I hit esc to quit the game I'm playing it stays black screened until I task manager out of it.
that is a different issue. not the same issue.

what do you mean by "refuses to play games from the external drive"? elaborate

If I play any game from a specific external drive it seems that they don't play while they're stored there. They crash seconds after loading or it will crash before the "Open Game" window even disappears. If I move that game to my Local C drive it will play them.

It's either the hard drive, or Dolphin's working when it wants to. My WiiWare is located on a different external drive all together, maybe that's why I don't have an issue with Wii games because my gamecube stuff is mainly on the HDD giving me issues. My HDD was defragged and scanned for errors and found none (plus the drive works fine outside of dolphin).
if you want to play games exclusively from your external drive, try installing dolphin on there. it could be a problem with trying to grab information from two seperate resources
