i'm not sure this qualifies as a major bug, but definitely fits the bill for annoying. has someone already made an issue report? if not i can go do it
Would like to know too, as I am getting the same behaviour since I updated Dolphin last week.
Sometimes the task manager shows up fine, sometimes not (black screen) and the only solution left is to reboot.
Just tried to revert to 4.0.2 (I always keep the last stable at hand) and everything works fine.
Tried today´s build, bug still there. Annoying boty (bug of the year)

Ok, so I bisected the issue. The commit that causes this is 4.0-6021 (Page 6 on Downloads) "Fix threading issues (PR #2295 from comex)", link to PR:

Steps to reproduce (did this with portable installation):
Select D3D Backend (OpenGL seemed to work)
Choose Use Fullscreen
Do NOT use borderless fullscreen
Start a game, and try to exit using either Esc or Alt+F4. Dolphin freezes, and only way to exit the program is using Task Manager to end the ("not responding") Dolphin.
Previous build 4.0-6018 works fine, the game starts and stops as expected. This seems to be related to D3D and exclusive fullscreen. How this is linked to a commit that fixes threading issues? Beats me.
If someone could verify this then I could make an issue report later.
People (like myself) experience this also when using OpenGL. Dolphin freezes when trying to exit a game (like when hitting the Esc key). Doesn't matter if it's in fullscreen or not. This doesn't happen to me when I'm using D3D. It will let me exit a game just fine without freezing up
