Running Dolphin, GC controller USB adaptor, in settings every button maps fine except for Axis Y- (aka main stick up). What's strange is if I move the stick freely all axis' appear to register fine in the visual graphic below -- it's just when I actually try and map it.
What kind of USB adapter are you using?
Does it work in game if you set it manually ( right click for manual mode )?

again what's strange is the free-moving visual below detects it moving in that direction -- so doesn't that rule out the stick and adaptor not picking up the signal?
someone told me they were having a similar problem and that it might be a mac-only issue?
Yes that rules out the adapter as an issue IMO. You could also test in the control panel and for kicks to a calibration may be worth a try. Could not hurt to hold up on the stick and screen shot for another pare of eyes to look but thus far I'm with you, its very strange.

this basically results in not being able to run forward in a game
and again even mapping manually doesn't seem to work, so strange!

From the pic it looks like it's working fine input-wise – is it just that you can't *run* forward, or are you unable to walk forward too? In any case, try fiddling with the radius.
basically can only walk forward, no running
which makes the game impossible to play
But you can still run backwards? If not, try decreasing (or increasing? I forget which way it goes) the radius.
