The fps counter still drops to 30 and back again to 60.

Yeah, in OLDs ishiiruka version drops 30 fps , in last version (250) not work again.
In New Super Mario Bros Wii or Newer Super Mario Bros Wii using efb to ram.
Graphics -> Render in the main window ... fixed times this. Not all times.
My PC: Intel Core I5 4690K , GTX 980 SC, 16 Gigas Memory.
EDIT: very times click in join game takes to display the log (messages on the screen) on the screen shows where the control of the Wii, if there is a delay (black screen before showing the wii control) is sure it will be falling fps.

Just playing the game.
On title screen, game starts with 30fps, on load game screen fps back to 60, in world map screen falls again into 30fps, enter a stage: 60fps again.
In world 3 (the one with snow) if you walk through the stages to reach the castle, the fps drops to 30 if you go up to the castle and you go back down again to the first half of the map.

You mean the patch that causes crashes in games including RS2? I don't think that'd be a good idea.
any side effects from that patch?
Oh, sorry. I didn't know that.
It crashes 3 stages of Rogue Squadron 2 and potentially other games in HLE audio. It works fine for RS3 though; so it's possible you could take the RS3 part of the patch without issues.
