I just tried 1108 revision and video works normal but warning message about missed extension still presents.
If everything works, you can ignore the message about missing extensions, especially for Pinned Memory since it's AMD-only.

Let me enter the game. Since the GLSL merge, all games are working half speed on my laptop (MacBook Pro, GeForce GT 330M). I am under linux debian 64-bit with latest nvidia drivers.

Let me know which tests I can try to help you pinpointing the problem.

Make sure your gpu isn't idle. Starting with the GLSL builds my gtx 660 was going idle in dolphin on Ubuntu, never happened in windows. I had to set it to prefer max power in the nvidia settings.
No, my GPU is at full speed...
ccox: so same process to indentify backend issues:
- Are you fill rate limited? (which performance do you get at 1xIR and 2xIR)
- Is it because of vertex management? (try hacked buffer upload)
- does cpu waits for gpu? (disable efb access and select efb2tex)

Same happens to me. FPS lovers to 30-35 for no reason, when switching to fullscreen
Yes, I am frame-rate limited: 1xIR ->12 FPS, 2xIR -> 7 FPS (twice this value with the pre-GLSL versions).
Hacked buffer upload does change the framerate.
I disabled efb access, but it does not improve anything. I have 12 FPS with efb2tex, 9 with efb2ram.
Your gpu does something wrong here. Better gpu would help, but I don't know why it's needed.

First try: Maybe it's because of uniform loads from cpu-ram. This patches upload all uniform to vram first:

Newer gpus have more cache, so that could be the reason why it wasn't noticed there
I tested the patch, but it does not improve anything, sorry...
