Optimal is 32x32 (for banner 32x96),but You can use different resolutions.

Yes, by putting a PNG file with either the same name as the game or the name "icon" in the same folder as the game. This was added a while before 5.0, so it won't work in 4.0.2 or older. It's also possible to edit the banner directly inside the ISO, but I'm not able to tell you how to do that.
I assume raganinjo was talking about the nobanner.png file that's used when there is no banner. It was part of themes in 4.0 but was moved to the Resources folder before 5.0.

Yes. Just like with editing the banner inside the ISO, explaining how to do it is too complicated for me.

They are only stored in the opening.bnr file that's inside the ISO.

The background for the game list is just a plain color. Dolphin doesn't support replacing it with an image.

Dolphin doesn't provide a way to do it. Either it isn't possible, or you have to change the color scheme of Windows – I'm not sure which.
