What version of Dolphin are you using?
DX11 does not work for SoA at the moment. So you will have to use DX9 or OpenGL.
Check if the custom projection hack is set up correctly (pictures below). If that doesn't help set external frame buffer to disabled.
I'm using Dolphin 3.5-367 x64, and still got the same problem. I set the custom projection hack to match yours, tested, done the same without external frame buffer, tested again and still got the problem. I don't have any logo from load screens too.
A couple of things to try:

1: Use the latest revisions; 3.5-367 is outdated. You can download the latest here: http://dolphin-emu.org/download/
2: Try using OpenGL. I spent around 50 hours on this game and didn't encounter any major issues with OpenGL.
3: Just a guess, but see if enabling "Skip EFB Access from CPU" and "Ignore Format Changes" does anything. It probably won't help, but I had them enabled with no issues.
Should I download latest stable version or latest dev version ?

EDIT : I just downloaded latest version of Dolphin and got everything solved ! No need deactivate anything because the game work smoothly without bugs !

Try with the latest or one of the latest development versions (currently 3.5-130X)
Ok the game works really fine, but now I face another problem : I got a really high drop of FPS when entering underwater areas in Mount Kazai (3-5 FPS max, ~15% speed)
By the way, when not moving, the game returns to 25 FPS (100% speed)
I'm using DX9 and I got the same drop using OGL (5-7 FPS max, ~40% speed)
Try manually setting the Framelimit (30 for NTSC, 25 for PAL) and check limit by FPS. If you can hold down the Tab button and get fullspeed or higher, this usually means it's an issue with your Framelimit settings; if you can't get fullspeed while holding Tab, it might be a hardware issue or other settings.

Try setting EFB Copies to Texture, if you're still using the settings you posted from the previous page. If you're still using LLE audio, try HLE instead. I played the entire game on HLE audio without issue.
I'm using HLE too, because I felt the overall slowdown due to LLE (even with a QuadCore Undecided )

This slowdown only happen when moving underwater. As I stated earlier, framerate return to normal when standing still underwater.
Does the slowdown happen in older revisions such as 3.5? Dolphin may have gotten a lot more demanding since I last played it. Most of the recent changes haven't solved any specific or game-breaking issues in SoA, so you should be safe playing with older revisions. Dolphin 3.5-78 was the first new-ax-hle merge, and HLE audio has been perfect (mostly) ever since then. Try using that revision.
I got the same problem with that version, so maybe it's coming from somwhere else.
