Not always. Though we do give off cues. Which are more obvious at younger ages (you will know when a baby is hungry for example). As our thought process becomes more abstract it is more difficult to determine from simple cues like body language alone.

What level of thinking? There are different definitions for this term. And yes we can determine this. Quite easily. Testing intelligence and sentience is just a matter of checking an organisms response to different stimulus.
You basically just asked me (without realizing it) "explain to me basic biology and then continue all the way up to graduate level neuroscience". The quick answer is we know the same way we know anything else. Data, logic, and the scientific method. We've done a lot of research into human psychology and neural networks over the years (particularly the last few decades). I suggest that you read up on some of it if you are truly curious about the subject. From what I can tell nothing that I posted is disputable within neuroscience at the moment nor did I mention being able to read peoples thoughts. I merely listed some of the observable steps from birth to becoming a sentient organism. Since these are observable events they're not really debatable (or at least they would require a mountain of counter evidence to debate).
