Not sure what you're doing there, you refuse to post settings and my game looks great all the time. There is not much more I can do here.

If I toggle the circled option just after starting the game, it works.
Those are not all of your settings. Anyway, lets forget that and do this:

1: Start fresh, Download the latest build from here --> http://www.dolphin-emu.org/download.html

2: Use these settings only: http://forums.dolphin-emu.org/showthread.php?tid=23618 but change your internal resolution to what you normally use (2.5x, 3x or 4x is fine) (use DX9, try with OGL later)

3: Do what post 8 says again

4: Tell me what happens

Followed your steps, the settings page has been modified since your post, but I didn't change anything that you didn't have set.
I'm not sure what else to tell you, try updating your video card drivers or use another video card altogether and see if the results are the same.

I know you're out of ideas, but here's what it looks like if I toggle Disable EFB right after starting the game: http://i.imgur.com/QK0sB.jpg

I suppose that workaround will have to suffice.
