Trust me daco if you lower the settings you'll have no problem at all, and there is so little difference between the settings besides draw distances and texture quality (which can be raised separately without any performance hit). The main difference between the settings (at least the only truly noticeable difference) is the draw distances. With max settings the mountains 5 miles in the distance will be rendered with the same quality as everything else, which will completely destroy your performance.

I've discovered so much in this last hour. This game is great but it does reek of consolitis.
Enabling ambient occlusion on nvidia cards (warning, it has a HUGE performance hit and should only be used on high end graphics cards):
TRSSAA can also be enabled with nvidia inspector and adds a lot to this game visually but has a huge performance hit (as you would expect).

EDIT: After much discussion in the thread, it would appear the above FOV command does not stick, and will revert to 70 on Fast-Travel. Try the console command "Fov xx" (~ for console, xx for an fov number)
Vsync is enabled by default and cannot be disabled from the in game settings, you need to modify the ini file.
Details about the bTreesReceiveShadows and bDrawLandShadows settings (these two settings are not enabled even if you select ultra settings despite the fact that they make the game look A LOT better in some spots and have virtually no performance hit what so ever):

Tweaking x and y mouse sensitivity separately:

It improves the tree shadows – You can change it to 1 to enable shadows on trees or keep the shadows disabled if you are looking to squeeze performance out of your machine.
bDrawLandShadows=0
It enables land shadows – You can change it to 1 to enable land shadows. Again, shadows eat a lot of system resources and video card memory, keep it disabled if you are looking to squeeze performance.
fGrassMinStartFadeDistance=0.0000
It improves grass distance. Greatly effects performance. I have tested it up to ’400.0000′ – you can tweak it this variable either for graphics or performance.
iWaterReflectHeight=512 / iWaterReflectWidth=512
It improves Water Reflection Resolution, which obviously will use your system resources. You can either keep it as it is, if you want to improve the performance or even lower it. Or change both to 1024 for eye candy.
bMouseAcceleration=1
If you want to disable mouse acceleration, you can change it to 0. You will need to increase the mouse sensitivity in-game when you disable the mouse acceleration.
uiMaxSkinnedTreesToRender=20
If you are experiencing low textures and other textures related graphics issues, you can tweak this variable to improve texture and skin rendering. You can set this value to 40 to start with. Or keep it as it is, if you are looking to squeeze performance.
Additionally, you can add following ini tweaks at the end of the your SkyrimPrefs.ini file:
bUseWaterReflectionBlur=1 bReflectExplosions=1 iWaterBlurAmount=4 bAutoWaterSilhouetteReflections=0 bForceHighDetailReflections=1
The above five ini tweaks improve water rendering – Again, you can keep either lower the value or set them as it is, to make water look gorgeous.

If you want to improve mesh on long distance in Skyrim, add the above three ini tweaks at the end of the configuration file. These tweaks will improve the mesh distance.

You'll want to set maximum prerendered frames to 0 to reduce stuttering on nvidia cards (and whatever the ati equivalent of that is called as well).

By habit I have always turned on VSync for the games I play and they have worked fine with no issues.
Why is VSync considered to suck ass?

I highly recommend disabling vsync and mousesmoothing over everything.
Vsync will lock your framerate to 60, and when it gets really intensive and your FPS drops, it'll spazz, and lock you to 30.
But indeed, the mouse is terrible in this game. I usually have around 70fps, but whenever I enter simple places, I get around 200-300FPS and the mouse starts flying all over the place.
Also enabling the tree shadows etc will improve the look of the game a lot.
But the game has bugged me. I can't switch my hand spells anymore. The right is stuck, only the left one changes... Also, the hands seem swapped, right-click = left hand and vice versa...
(I had to switch the bindings to make it natural again, but ingame it obviously states the wrong hands)
If anyone knows how to fix this, please let me know.

So that's what VSync does. I guess I wouldn't even notice a difference if I switched that off.

or it'll spaz and always lock you on 30 even if your system can handle 60fps fine
which sucks cause my eyes are used to play games > 30fps meaning its not fluid to me. i hate that.
or imagine a game like unreal tournament which natively runs 60 or more. lock that to 30fps
