Ya I am thinking I should do the main quest first before I start using mods. and such. However it is a bit annoying that I have to set everything to medium to play at a decent speed -.-
Well, you do have a very slow GPU. I think its high time for an upgrade.

Vsync is Vsync, Lag is a given unless you triplebuffer as well as framecap to a value just below the refresh rate.
Ya I know... it was better then what I had before it was an AMD Sapphire 512mb (
http://www.sapphiretech.com/presentation/product/?psn=000101&pid=311&lid=1) and that struggled to play portal 2 :O... like when if you were to place a portal on the ceiling and on the ground so you fall through and build up momentum it would struggle. My current card can play most games I like - I am some what shocked it managed to pull of Mirrors Edge on max settings... any way to stay on topic I defiantly need to upgrade my GPU - I could have swore it was crying when I tried to run the game on max settings :p

I know what VSync is. But for some reason, when you have VSync enabled, the game gets some weird lag which usually happens in caves/dungeons etc.
The mouse starts moving with delays, feels like 'jelly' and the screen gets jerky.

The difference between max and medium isn't that big anyway. You'll only get to see more shadows in the distance and better detailed terrain.
Feel free to post screenshots here aswell.

I'd recommend at least getting the two released volumes of sky sounds or the sounds of Skyrim or whatever they're called, and the fall of the space core vol 1, as the sound mods don't affect gameplay, yet make it more immersive, and the portal mod was half done by valve and half done by Bethesda (so was wanted by the developers), and also doesn't affect gameplay negatively.
What's your performance?
It looks like you have AA on, try to switch that off and use FXAA instead. It does blur the image a bit.
Here's one I took a sec ago.

I assume that FXAA is a nVidia only thing?
