Well, assuming you're talking about a d3d9 game, run something like fraps in the background to give you a framerate counter in game. Try to use settings that would normally give you a framerate above 30 but not usually above 60. V-sync will cause your framerate to become locked to either 60/30/20/15 so you'll see it pop between those and nothing in between.
Another easier way to tell would be to look for horizontal tearing, if you see any that means v-sync is not working.
This is all assuming you are using a 60hz LCD monitor.
