I get a similiar problem, except I set up my mouse and keyboard as emulated wiimote (slot 1) but once I start up a game it acts as if it's the 2nd.

Weird.
I think I might have a temporary fix, but it isn't perfect.
When in game, Change all 4 Wii Mote slots to off. Refresh, and then reconnect.

I often have sync problems too, but they usually fix themselves after retrying over and over. Are you syncing your wii remote before or after loading a game? I would try both; usually connecting them before works better for me.
Also try turning off the wii remote manually, hitting refresh so that there aren't any remotes, and then retrying.
If that doesn't work I would try connecting the remote manually through your bluetooth manager.
Next, I would try syncing the wii remote to a wii, then turning both off and retrying with Dolphin.
Finally, I would reboot my computer.
