If you have the Item hang patch selected and are using LLE and still get the issue, try version 3.0. I can personally attest to it working.

As far as I know, this issue only occur with DSP HLE if it gets desynced or muted... Generally, disabling audio trottle and setting framelimit to 30 solves this issue on DSP HLE (this issue doesn't occur on LLE)...

You're right the official release is perfect for this game

I agree for Audio Throttle. Limiting Framerate isn't essential : I've never done it and it works too
