There's only ever one ghost ship. Once you get the Triforce chart from it, you can never find it again and the chart's useless.
I'm not exactly sure if the moon phases are still supposed to show up, though. They still show up in Wind Waker HD, but not on the original in Dolphin. I'd check this on real hardware, but I'm not gonna go plug all that shit in again with a wimpy little TV like the one I currently have.
