Wiimote Plus + Skyward Sword POST WORKING HARDWARE CONFIGS ONLY

I have done about 4 hours of reading tonnes of mixed reviews about what works, what isn't working and everything in between for the Wii remote plus's, but have yet to see anyone post THIS SET UP WORKS. If you are one who has the Wiimote Plus working with your PC please post:
-type of Wiimote Plus controller and specify model (ex. RVL-CNT-01-TR) and if you are using a nun chuck
-Blue Tooth receiver and stack
-Dolphin revision and any mods that you have done to it, if modded please upload your revision
-any abnormal procedures you need to preform in order to successfully connect and utilize the controller in game.

I'll just leave the one fact I know here: the golden Wii Remote that comes with the game has MotionPlus built-in, isn't a -TR remote, and works absolutely perfectly with Dolphin on any BT stack that works with a normal old Wii Remote. Use that one if you can, it's wonderful.
