Set the wiimote plugin to emulate swing instead of pitch and roll to get past that section. You'll be able to do a full 360 rotation.
Simulate a swing on the z axis to hammer.

BTW darknight - did nosound97's movements work for you without having to configure anything?

como se menciono antes tenes que entrar en configuracion de dolphin y ahi entras en el pugin de wiimote y le hace boton map y combias la de auntorotacion a swirt entonces solamente al momento de que tengas un paciente con cardia agarrest debes apretar intercaladamente los botones arriba y abajo de roll de wiimote
