With 4x IR, the image actually has to be stretched up to match your monitor's resolution, which results in poorer image quality than trying to have an image shrunken down to match your resolution. If your IR is set to Window Size, your IR is your monitor's resolution, so yes it's 3840x2160 in your case.

No. 4xSSAA generates an image 2x the width and 2x the height of the original for 4x as many pixels in the result (2 times 2 is 4). 16xSSAA would give generate a 15360x8640 image (4x the width and 4x the height) giving you 16x as many pixels as your 4K resolution. So in other words, IR set to Window Size + 4xSSAA forces your GPU to compute an image that's 7680x4320 (also called 8K).
