Issues unfixed that i know:

Bloom /offset(many games)
Mario kart double slowdowns

Metroid prime random shit:

I will run more tests in diferent games and see what i got.

Also slowdowns on Super Smash Bros Brawl, like luigi slowdown the game to hell, and also happens when certain characters or effect spawn on screen.

Game runs fine only slowdown when certain elements appears, like playing with luigi slowdown, i see one pokemon spawn that slowdowns to, and zelda boss or something.
And when they are gone, game back to normal.

Metroid prime issues are strange maybe is related with efb to texture in this build.They auto fix fast, and i noticed only one time in efb to ram.

Neobrain has made a workaround for the costum texures issue. It does fix my issues with SMS (also no slowdown with the sand texture). Maybe you can implement it or it helps you find a fix for DX9...?

StripTheSould: I'm trying to get acces to a friends computer with a nvidia card because i can't reproduce the issues you show me in dx9, textures works perfect on my machine.
Hyorus: It was a big texture that was recreated every frame instead of reused

Forget the metroid prime texture issues, is related to efb to texture, and fixed with efb to ram, also happens on 4.0.2 and this build.
