1. Enable the Item-Hang-Fix (Right click on Iso -> Properties -> Patches -> first option), then start and get after the cutscene. Save and quit Dolphin, uncheck the fix, and play.
2. That should not happen normaly. What version do you have? GC or Wii? The Wii-Version worked fine for me...
Just try to start it again. I don't think the TLBHack-Solution is the right for that...
