In DS4Windows your Dpad is listed as Right > Right, Left > Left, Up > Up, Down > Down, so your DualShock should be mapping to an Xbox controller's Dpad. However in Dolphin you have G, F, H, and I'm assuming previously T. Those are the default keyboard controls for the dpad, so I'm assuming you forgot to map them.

I also spotted one small issue. You mapped L and L-Analog (and the same for R) on separate buttons. Just map the analog and digital versions of each button to the appropriate triggers, making them the same. That is to emulate the dual-action analog then click than the GameCube controller triggers have.

Also remember to calibrate your joysticks! It'll work without it but it will work better with it.
