Anyway I eventually got it working thanks to your suggestion. I tried binding to keys on the keyboard but the digital input just didn't work properly and it would get centred regardless. Direct mouse axis did the same thing.
tl;dr, set up the CC as normal (set Device to the 'pad), then right-click each button for the mouse controls and set the Device at the top of that subwindow to the DInput Keyboard/Mouse, then choose a control in the list and click "Select". (Also, don't choose the Axis controls for mousing with, choose the Cursor ones. Otherwise it'll keep snapping to center, which obviously doesn't work well.)
Eventually I noticed cursor axis at the bottom of the list and that worked perfectly. It's clunky as hell but it works.
