Sorry, I can't understand you. When using vsync ON and framelimiter OFF, the game is perfectly smooth (but only playable with muted sound, because sound it's distorted and accelerated). So, dolphin vsync works well.

The game you are playing is a 30 fps game then or has variable fps, accelerated sound is normal since you are running it double the normal speed with vsync enabled and without framelimit (i suppose your monitor has 60 hz rate). In case you still don't understand just use framelimit auto and vsync for better experience.
