Uninstall it from the NAND first before running the patched WAD. You need to enable Dolphin's very own Widescreen hack as well.

That did it! I did not know the widescreen hack had to be enabled for it to work, because you don't need it with the Gecko code.
Do you happen to have the AR code handy?
I do not see one in the texture pack thread or the wiki, only Gecko ones.

Thank you! Awesomeness has returned to SM64 on Dolphin.

To be honest, I'm not sure what the AR code actually does because I don't see a difference with or without it, but the widescreen hack does certainly take care of business. I will leave it enabled in case it affects things I haven't noticed yet. Only the file selection menu remains misaligned compared to Gecko codes, but this is a small compromise in order to be able to play 60FPS and widescreen.

The AR Code (as well as the Patcher64+ option) removes the culling that occurs through Dolphin's very own widescreen hack. Aka, no stuff popping in and out in the extended widescreen view area.
