Ah good old Mayflash. It's very well know around here that mayflash has had their fair share of issues. However I do believe they have fixed most of them via driver updates. Your first step would be to contact them and get the latest drivers. If I understand that last post right, ps2 vibrate is ok and GC is the one with troubles? If so its not a dolphin issue. To make a psx shake you send data to the controller, to make a GC shack you flip a bit in the poll string. Meaning GC is on or off, psx is a bit more involved. Looks to me like mayflash had a bug where they didn't turn off the GC controller (hence it does not stop). I think all of your issues can be resolved by contacting them.
