Hello I am having a similar issue.
After receiving my dolphinbar and updating firmware only mode 1 has any response.
I've tried the fix listed above by Turtlelogic but no change.
I can see all 4 modes as I switch through them being listed in Devices and Printers but when I go to Dolphin Wiimote setup, under Real Wiimotes it reads "A supported bluetooth device could not be found. You must manually connect your wiimotes".
I have selected "Real Wiimote" at the top beside "Wiimote 1" and seeing as how mode 1 controls my mouse pointer fine I believe it to be synced correctly. I continue to try and refresh the detection of a bluetooth device in the Dolphin Wiimote Config but still no joy.

PS. I am using Dolphin 4.0.2 and am attempting to play Skyward Sword. I recently received my Dolphinbar along with an attachable Wii Motion Plus with sleeve to use on my first gen wiimote + nunchuck.
