I recently noticed that Dolphin never goes into exclusive fullscreen.
(Direct3D 11 never shows a message about exclusive fullscreen, my screen doesn't blink when I exit fullscreen mode, I don't see any tearing even after disabling vsync... so I'm pretty sure I'm in borderless window mode although that option is disabled in the preferences)
I have two GPUs (Nvidia 820M and Intel HD4600), my guess is that Nvidia Optimus doesn't let me go into the exclusive mode.
I have all drivers and Dolphin updated, I changed some options, I tried to force the use of one or the other GPU... but I couldn't accomplish anything.
Do you have an advice for me?

Video Card/GPU: Nvidia GeForce GT820M 2GB GDDR3
Memory/RAM: DDR3 4GB
