I had the exact same issue, here's what I did.
1. Use Zadig and install LibUSBk driver vor BCM2045A
2. Install the latest x64 usbdk driver as advised in the wiki https://wiki.dolphin-emu.org/index.php?title=Bluetooth_Passthrough#Forcing_a_specific_adapter
3. force adapter to be used after converting to Dezimal. For me the decimal values are: VID = 1406 and PID = 773
4. Don't use the latest Build, it seems to not work. I use 5.0-2341 and it works fine now.

Tell me if you got it wokring using these steps.

Has anyone attempted to house this chip inside an official Gamecube USB Adapter? It occurred to me that piggybacking off the second USB plug (which is presumably power only out of the factory) and wiring the BT module in parallel would make the perfect adapter box.

I finally feel confident enough to attempt this, and have an original module lined up in a month or so, but thought I'd throw the idea out there to see if someone else can confirm this is reasonable.

I actually found a pretty good 3D printer kit on Amazon for £120, so it's already cheaper than buying a few things from somewhere like ShapeWays, and might be worth your money if you're going to print more than just this one box.

I successfully built this bluetooth module and the wiimotes work perfectly, but I'm not able to make the Mayflash Dolphinbar (sensor bar) work. When I install the driver from the latest installer Zadig asks where I want it installed? How do I get this dolphinbar paired to the BCM2045a chip? Think this is what needs to happen.

That isn't possible.

Depending on the firmware and hardware revisions of the DolphinBar, it might be shutting off its IR LEDs while its own Bluetooth isn't in use, making it impossible to use its sensor bar functionality while you're using a different solution for Bluetooth.

OK, so how is everyone getting the dolphinbar to work? I've got the wiimotes running over the custom bluetooth module, but am stuck on how to get dolphinbar working. What am I missing?

You don't.

The Dolphinbar is 100% incompatible with this method of connecting the wiimotes.

If you want to use the Dolphinbar, do not turn on bluetooth passthrough and just set controllers to "Real Wiimotes", set Dolphinbar to Mode 4, and press the sync button on the bar + wiimotes.

If you want to do this, you need a USB powered sensor bar. The sensor bar is just a bunch of infrared lights for the wiimote's IR tracker camera to track. The Dolphinbar is designed to shut off the IR lights if nothing is synced to it, which makes it unusable for this.
