Another help I found in RS2/3 as of late is enabling "Progressive Scan" in the graphics settings. It made the game run much smoother even if it said I was getting 60fps. Seems like a few months ago the game started getting really laggy, but that ended up being the fix for me.
I will certainly have to try out the idleskip option too and see if I can get RS3 running smoothly.
