GC controller not recognized by direct input

I'm using the Mayflash GameCube Controller Adapter for PC USB. The controller inputs all work perfectly when tested via the USB GamePad Properties (via USB controller setup) on my Windows 8.1 laptop. However, there are issues with getting it to function correctly with Dolphin.

I really only need it to work on the Dolphin 4.0-652 build (Super Smash Bros. Melee netplay), but I've tested it on the more recent 4.0-4873 build as well.

Here are my specific issues between the two versions of Dolphin:

4.0-4873: Does not recognize the adapter. Under the 'GameCube Adapter' header of the Controller Configuration window, there is grayed out checkbox with "Adapter not detected" next to it.
DInput: No inputs are detected.
SDL: No option for SDL.

4.0-652:
DInput: Inputs not detected. Hitting refresh causes Dolphin to freeze.
SDL: Inputs are detected, but there are significant issues that didn't exist when tested via the Windows game controller setup properties, including input delay, drops, and imprecision (specifically with the L and R buttons).

I would like to be able to use Direct Input, since I have heard others using it for the same build have better input detection.

I'd appreciate any help. Thanks in advance.
