Hi,
I am using the Mayflash Dolphin Bar to connect 2 Wii Controllers with Nunchuks.  The Dolphin Bar is set to mode 3, Game Controller Mode.  The controllers connect properly, and I can confirm that they are both working in the Control Panel.

In Dolphin Controller Configuration, GameCube Controllers, Port 1: Standard Controller, I can select either controller and they both work properly:
DInput/0/Mayflash Wiimote PC Adapter
DInput/1/Mayflash Wiimote PC Adapter

In Dolphin Controller Configuration, GameCube Controllers, Port 2: Standard Controller, I can select either controller but all controller inputs are disabled.

Any suggestions would be appreciated.  It would be great to get my second Wii controller working as a GameCube controller.  I can use Joy2Key as a workaround, since it successfully detects input from both controllers, but there are some limitations with that approach.

As an update, I've realized that this problem doesn't have anything to do with the specific controllers I'm using.  I've tried many combinations of different types of controllers.

Dolphin never registers any controllers under GameCube Controllers Port 2.  My controllers all work fine under GameCube Controllers Port 1, but somehow the other ports are disabled.

Has anyone had a similar problem?  Any ideas would be appreciated.

I was able to get this working. I just needed to select the 2nd controller, and then add all the buttons/axes to the configuration. For example, I clicked "Up" then pushed up on the controller up. Then I clicked "Down" and pushed the controller down. And so on for each of the controller's axes and buttons.
