When using frame advance, it takes the emulator longer than normal to advance a frame. When holding down the frame advance key, it advances a frame about every 2 seconds. Sometimes after letting go of the frame advance key, the game will begin to play at full speed rather than staying on the current frame.

I tested this on 4.0-8684, 4.0-8711, and 4.0-8717. This does not happen to me on older revisions such as 4.0.2.

I assume this doesn't happen if you're pressing the key rapidly? You can adjust how many frames per second Dolphin runs at when the key is held with the hotkeys Frame Advance Increase Speed, Frame Advance Decrease Speed and Frame Advance Reset Speed.

This is a known problem. The cause isn't known yet, but it will hopefully be fixed before the release of 5.0. https://bugs.dolphin-emu.org/issues/8718

For now, the recommended way to avoid it is to use the older version 4.0-5400. https://dolphin-emu.org/download/dev/c18c50a0e103db76da1834855efd4e136953afe7/

I get the feeling the hotkey merge caused more problems than it fixed. why wasn't it reverted?

Because the new functionality was desired. Maybe another factor was that the broken things weren't noticed by most users (the beeping only happened on OS X, frame advance being super awkward to use almost only affects TASing...)

But yes, you are right about the hotkey changes leading to a lot of regressions. In my opinion, it's the biggest source of annoying regressions to track down before the release of 5.0.
