This is a good idea. I also personally find the scanning of the entire library quite annoying (seeing as I have tons of games in there & it takes like 20 seconds every time) every time something is changed there. If it was dynamic/realtime & showed the appropriate banners immediately instead of refreshing, that'd be a nice improvement too.

Not very crucial for the gameplay on dolphin itself, but would make the emu nicer to use. It's in fact the only emulator I know that uses such a scanning process. I prefer the one in project64, which reads all ROM info from your folder and lists it immediately realtime, if you press refresh it automatically adds/removes items that have been added or deleted

Now that I think about it , this is what the refresh button in the GUI should do , refresh a already cached ISO list ( considering that the list is refreshed every time you open dolphin it is kinda useless )
MB : MSI Z68A-GD80
CPU : Intel Core i7 2600k @ 3.4 GHz (stock clock)
Gfx : ATI Raedon HD 6850
Ram : 4 GB DDR3-1333
OS : Windows XP x86
