DS4 Controller not working

I plug in my Dualshock 4 controller, open DS4Windows, choose the Xinput and click refresh in Dolphin controller settings and when I try to change a key it just doesn't respond. I move my mouse and press a button on my keyboard and it just changes to Keyboard and Mouse settings. I tried using Xinput and DS4Tool and nothing seems to work. My DS4 controller is fine on PC games but it doesn't respond on Dolphin.

How do I fix this?

I second this. DS4 NOT working with latest version of Dolphin. I'm using the OSX version nightly (5.0-522). I'm on the latest version of El Capitan and other apps work perfectly with the controller, i.e.: OpenEmu recognizes the controller properly as "Sony Playstation 4 controller". So I don't know what your guys problem is but this needs to be fixed soon. Also, why is it even necessary to show the full path of the device? Just show the proper name of it like every other app in the world. Thanks.
