When sorting by name, Dolphin always uses the name that is in opening.bnr, which sometimes is different from the name that actually is displayed. With your 400 MB ISO, it probably can't read opening.bnr at all, giving the game an empty name for sorting purposes. This will be fixed in an upcoming development build called "DolphinWX: Fix sorting games by custom titles". I'm not sure if Dolphin actually sorts by names by default, though, so you might want to check that first.

The hex editor I use is HxD, if you'd like to give it a try. I haven't had any problems with it, even when editing 8 GB large Super Smash Bros. Brawl ISOs.

Ah, makes sense. It actually defaults to the 'file name' if it can't read the opening.bnr and just throws it to the top of the game list.

Look forward to the upcoming custom title improvements.

The issue with HexVUI32 is that it's a 32-bit program, so can't load the whole ISO into memory. Any 64-bit hex editor should be fine, though.

Why would a hex editor need to load the whole ISO? Only loading data that's near what the user is viewing uses much less memory and reduces the time it takes to open a file.

I'm aware that it's silly, but I tried three 32-bit hex editors before I realised that's what they were doing and switched to a 64-bit one.
