Only thing I can really suggest is that you try upgrading to Dolphin 5.0 when it is released.  There is a good chance your issue might be fixed by that or subsequent builds.

Dolphin 5.0 basically will be a fork of Dolphin 4.0-6727 with lots of bugfixes to make it really stable. In other words, if the issue occurs on current development versions, it'll probably happen in Dolphin 5.0 too (at the time of this post I haven't seen any bugfix commit in stable branch aiming at this particular issue but that may change until final release)...

Looks like this is a known issue: https://code.google.com/p/dolphin-emu/issues/detail?id=8627 (Issue 8627)

Thankfully I've found out that I can just close Dolphin and then unplug and re-plug in my USB audio adapter to get my sound working again (as opposed to restarting every time). Looks like I"ll just have to wait for a development version to fix this issue.

P.S. - JMC I can see you posted today in that thread, the issue still persists, I tried out the most recent version (4.0-7320), and I still get no sound.

Edit 2 - I downloaded a much older development release to see if the issue is noticeable back then (4.0-3868 - ~9 months old), and I get completely working sound. So I'm not sure at which point something was changed that caused the issue, but this makes progress and hopefully helps isolate the issue. (updated original post)

This issue started when support for the official Wii U GameCube adapter was implemented...

Bumping this issue, still have the problem in the latest dev release (4.0-7420). Sound works perfectly in older 4.0-5400.

Turn off direct connect if you haven't. That seems to fix the issue for everyone who has it. Regardless, if it's related to LIBUSB, there's a very good chance it's not Dolphin's problem and you should be complaining about someone else's drivers/hardware.

I have disabled direct connect, and unfortunately it doesn't solve the problem.  Going back and testing prior revisions, I have pinpointed the exact revision where sound is broken for me.

-Sound works perfectly on 4.0-5402
-Sound becomes broken on 4.0-5406

Were there any major revisions to the audio section of Dolphin during 4.0-5406?  According to the note on the download page, the revision just fixed some hotkeys, thanks.

No. I mean, I still think it's related to LibUSB, since that's the only thing causing issues for other people in various setups/drivers; I just don't know how to pinpoint it if a different build than usual is causing it.

Yeah I don't know what the problem is, I just know that the problem starts happening on build 4.0-5406.  I've been in contact w/ the USB adapter company, and I haven't had any issues with any other programs except for Dolphin (all PC games, media, other emulators, etc... work fine).  The fact that an older Dolphin revision also works fine leads me to believe that it is some kind of change that was made in Dolphin 4.0-5406 and newer.  I don't have any idea of how to address that though, I just leave it to you guys as the experts.

Bumping as the problem still persists in the latest dev build (.7561). I'm still unable to get working audio via my USB device past dev build .5402, it is broken on .5406 and forward. Could thread be moved to the support forum, would appreciate any help thanks!
