Another contribution, tested in Dolphin 4.0.1 too...

From: Jhonn
Dolphin: tested in 4.0, 4.0.1 and latest master branch build
Wiimote: Wiimote without Motion Plus Integrated (RVL-CNT-01), Nunchuck and Motion Plus Separate Accessory and a non-TR Wii Remote Plus
Brand: Genuine Wiimote and Nunchuck from Nintendo. Fake (unknown manufacturer) Motion Plus Accessory. The Wii Remote Plus is from Nintendo too (got it from FlingSmash bundle)
Bought from: Local Reseller
Bluetooth: Qualcomm Atheros AR3012 Bluetooth 4.0 + HS
Bluetooth ID: USB\VID_0CF3&PID_3004
Brand: Atheros AR5B22 Combo Card (mine was manufactured by Samsung)
Bought from: Local Reseller
Sensor bar: DIY bar, 10 IR LEDs (5 at each side)
Bought from: Home made
OS: Windows 8 x64
Working: Everything works fine in all games that I have, even in multiple Wiimote setups (tested with 3 at the same time)
Not working: Sometimes the external Motion Plus accessory aren't detected (probably because it's a non-genuine/fake one)
Notes: The Atheros stack doesn't replace the default MS stack (they work alongside) and this combination gives me more functionality that any other stack I tried (and the MS stack still works fine with continuous scanning from Dolphin, for example). Said that, always use the Atheros + MS stack unless you have -TR wiimotes (you'll need the Toshiba Stack in this case)...
Avell A70 MOB: Core i7-11800H, GeForce RTX 3060, 16 GB DDR4-3200, Windows 11 (Insider Preview)
ASRock Z97M OC Formula: Pentium G3258, GeForce GT 440, 16 GB DDR3-1600, Windows 10 (22H2)

[ *** color=#32CD32]
any thing in here will be the color green its RGB #rrggbb So for solid red you do #ff0000
Also remove the ***'s I did that so it would not work (for demonstration).

Dolphin: 4.0.2
wiimote: Remote with MotionPlus & Silicone Sleeve + Nunchuck Controller Set for Wii - Blue (2 x AA)
brand: New2in1 (or unbranded? unsure)
bought from: dx.com / dealextreme.com SKU: 70788
Bluetooth: Internal Qualcomm Atheros
Bluetooth ID: Unsure how I get this. Help and I'll update.
brand: Qualcomm Atheros
Sensor bar: Bought from ebay long ago, can't find.
Bought from: ebay
OS: Windows 8.1 64bit
Working: Everything so far. Been playing Zelda SS for hours without issues.
not working: I bought 3 controllers, one of them wouldn't connect. I haven't tried connecting it to a real Wii yet to see if it's faulty or just a different technology.
notes: I followed this guide.[/color]

First edit: If you have problems with this specific controller, feel free to PM me.

Second edit: I've received a couple PMs from users who has been unable to get this controller to work with Dolphin, even after following the guide. It could be that the controllers they sell aren't identical, or that their computer doesn't support the controllers. Buy at your own risk.
