Well I already have contributed to a thread like this but my post are gone now (not sure why). So, here is my contribution to this list:

Brand: Genuine Wiimote and Nunchuck from Nintendo. Fake (unknown manufacturer) Motion Plus Accessory

Bluetooth: ISSC EDR Bluetooth Adapter

OS: Windows 7 x64, Windows 8 x64
Working: Tested only with one remote connected (can't test with more wiimotes). Everything works fine in all games that I have, as expected
Not working: Widcomm bluetooth stack aren't compatible with this BT adapter (MS, BlueSoleil and Toshiba stacks works fine, tested all of them)
Notes: If you don't have the TR model, ALWAYS use the MS stack: the other stacks gave me poor results compared to the default MS Stack that come in Windows 7/Windows 8
