depending on how gcpad handles guid's and control devices (Di doesn't support hotplugging (unless you use polling hacks) while xinput does), the game might have been trying to make use of a controller that didn't exist

True. I have this problem with no USB devices plugged in. As soon as I start Dolphin, regardless of the version (e.g. 3.0 old/new or 3.5 old/new), I get Clearing Code Cache and an immediate crash. I'm starting to wonder if my Intel Integrated Graphics controller is the issue. I'm also running on Windows Vista x86.

It's likely to be your problem indeed. Your processor is also too weak for Dolphin

I'm sort of having the same problem. I'm on an iMac 27" from Mid-2010, and every time I start up Mario Kart Wii, it says "Clearing code cache." at the top of the window. And it stays like that forever. Once I left it for a full 24 hours, and it was still there.

We don't support Dolphin 3.5 anymore. Please update Dolphin to 5.0 at a minimum, then make a new thread (this one is almost 5 years old) if your problems still persist.
