OS: Windows 8.1
Dolphin Build: 4.0.4875
Wiimote: TVR-01 Official with Mayfair DolphinBar
Issue: Happens during various times, especially when the game audio starts to freeze up.
WhoCrashed report below:

Quote:On Wed 12/31/2014 7:41:07 PM GMT your computer crashed
crash dump file: C:\WINDOWS\memory.dmp
This was probably caused by the following module: usbohci.sys (usbohci+0x1367)
Bugcheck code: 0x1E (0xFFFFFFFFC0000005, 0xFFFFF800E85DA367, 0x0, 0xFFFFFFFFFFFFFFFF)
Error: KMODE_EXCEPTION_NOT_HANDLED
file path: C:\WINDOWS\system32\drivers\usbohci.sys
product: Microsoft® Windows® Operating System
company: Microsoft Corporation
description: OHCI USB Miniport Driver
Bug check description: This indicates that a kernel-mode program generated an exception which the error handler did not catch.
This appears to be a typical software driver bug and is not likely to be caused by a hardware problem.
The crash took place in a standard Microsoft module. Your system configuration may be incorrect. Possibly this problem is caused by another driver on your system that cannot be identified at this time.

I'm pretty sure my drivers are up to date; have been updating them all along with chkdsk/reg cleaner actions.

Thanks for any input.

I started to get this problem last night quite frequently, would just keep BSODing on me. I figured it was something to do with the mayflash dolphin bar so I somehow managed to fix it using these steps:

1) check windows update.
I found that there were a few windows updates that had not installed correctly including a .net framework update. Flushed my Windows Update cache and re downloaded them.

2) I have a Logitech gaming keyboard with a screen that sends cpu and ram info to the LCD. I disabled the Logitech gaming app on windows launch so no extra data was being sent over usb to it.

3) removed my desktop background.
This probably didn't help at ALL but I did it anyway just in case.

4) rebooted the PC with the mayflash bar unplugged. Once windows had loaded, plugged it back in and ran dolphin. tested it for several hours with not a single BSOD happening after that.

It seemed to me that whenever I was using a real Wii remote, it was causing the machine to crash. I think if anything, step 1 and 2 might have helped the most.

Hope this helps anyone having problems with this particular error.
