I have Vsync enabled in my Dolphin settings (and render to main window), but I'm still getting some really horrible vertical tearing. I don't have it enabled in my Nvidia Control Panel (though I do have triple buffering enabled). Is this a common issue, or should I recheck some settings?

Does it also happen under D3D?
Rig 1: Windows 10 Home | AMD A6-1450 @ 600/1000/1400 MHz | AMD Radeon HD Graphics 8250 | 4GB RAM | HP Pavilion TouchSmart 11.
Rig 2: Windows 10 Pro | Intel Core i7-2640M @ 780/2800/3500 MHz | Intel HD 3000 Mobile | 8GB RAM | Dell Latitude 6320.

No, it does not happen under D3D, but I would much rather use OGL on account of my Nvidia card. D3D stutters like crazy for me. Should I upload my config.ini and graphics .ini?

Bumping this with settings, just in case. Both regular and graphics settings included.

If you read the July Progress Report, the addition of exclusive full screen to the D3D backend is supposed to improve v-sync performance, which is probably why you get less tearing in D3D than OGL. Exclusive full screen is a D3D concept, though, so at present v-sync simply doesn't work as well in OGL as it does in D3D. You can either use D3D and enjoy the lack of tearing while you play in slow motion, or you can use OGL and enjoy full speed while you see lots of tearing. Perhaps one of the developers will find a way to make this functionality work in OGL in the future, but it hasn't happened yet.

OpenGL actually got exclusive fullscreen enabled, but render to main window causes it to not work. It's just the user needing to not use render to main window, I think.

Would you happen to know which revision got that feature? Maybe Kurausukun could compare with the version before that to see if it makes a difference.
